Each full time, year-round, permanent resident (over the age of 3 yrs) of the Hills Village North Master Association must have a validated PHOTO-ID Recreation Badge if they are using any of the Hills Village North Master facilities. This includes use of the: GYM, TENNIS COURTS or the POOL. There are no exceptions to this rule.
Please read through the following procedure to obtain a Recreation Pass with a current validation sticker. By filling out this form you will be registered into the database and a validation sticker and guest coupon booklet will automatically be sent to you each year.
A) If you have a photo I.D. badge(s) from a previous year and ONLY need a sticker to validate it for the current season. Please follow these instructions:
1. Complete the information on pages 1 and 2 of "Procedure to get Recreation Badges" form.
2. Submit two (2) proofs of residency for each person. This can be a CURRENT copy of a driver’s license, lease agreement, purchase agreement, personal check, or utility bill. For each child between 3 years and 18 years of age you will need to provide a copy of a birth certificate evidencing that you are their parent.
3. Drop off or mail the information to the property management office located at the above address. Please allow at least 10 days for processing. BADGES WILL NOT BE MADE WHILE YOU WAIT AS PROCEDURES REQUIRE ACCOUNT VERIFICATION. Your validation stickers and guest coupon booklet can be mailed out to you or picked up.
B) If you DO NOT have a photo I.D. badge from a previous year: You will need to mail or drop off the following to the onsite property management office so a recreation pass can be created for you:
1. Complete the pages 1 and 2 of "Procedure to get Recreation Badges" form.
2. Submit two (2) proofs of residency for each person. This can be a CURRENT copy of a driver’s license, lease agreement, purchase agreement, personal check, utility bill or piece of mail showing the name and address for each adult residing in the unit. For each child between 3 years and 18 years of age you will need to provide a copy of a birth certificate evidencing that you are their parent.
3. A small “headshot” photo of each resident must be submitted. The photo must be approximately 1 inch high by 1 inch wide. Please do not submit anything smaller than ¾ inch. Do not cut out photos – they will be cut in the office. There is no need to have a special photo taken for this. You may send any snapshot as long as the person’s face can be cut to a 1” x 1” size. You must sent a separate photo for each resident. Please submit the information on page 2 and 3, along with the above items and return it to the property management office by mail or drop off. Please allow at least 10 days for processing.
BADGES WILL NOT BE MADE WHILE YOU WAIT AS PROCEDURES REQUIRE ACCOUNT VERIFICATION. If all required information is received, your badges with valid stickers and guest coupon booklet can be mailed to you or you can be called to pick them up at the above address.
